Thank you for choosing Bluebell Physiotherapy.
By booking an appointment or having treatment with Bluebell Physiotherapy ("Bluebell Physio", "we" or "us"), you agree to these Conditions of Treatment ("Conditions"). Bluebell Physio has employees and independent contractors ("our Team"). As a customer of Bluebell Physio, you are described as "Patient" according to this agreement (also "you" or "your").
Our Privacy Policy explains the way we collect and use your information. By using our services, you agree to be bound by these Conditions and our Privacy Policy.
Please take some time to read this information so you are fully informed and have the opportunity to ask questions if required. Some of your questions may have already been answered in our Frequently Asked Questions, or if not, feel free to contact us.
These Terms and Conditions form part of your consent that allow us to provide you treatment.
1. Treatment
1.1 I give my consent to receive a physiotherapy assessment and treatment, including the necessary examination and physical contact in the relevant area(s).
1.2 I understand that procedures and treatment options will be explained to me and I will be given the opportunity to ask questions.
1.3 I understand that I may withdraw my consent at any time.
1.4 I understand that if consent is not given or withdrawn, Bluebell Physio will not be able to carry out any further treatment.
1.5 I am aware that I may bring a chaperone with me to the appointment should I wish.
1.6 In the event that a member of our Team is off sick or away, I understand that I may be seen by another member of our Team to avoid interruption to my treatment.
2. Funded or Referred Patients
For patients whose treatment is via a referral or funded by a third-party, you also agree to the following:
2.1. I understand that as part of my treatment, I consent to allowing any reports regarding my assessment and treatment to be released to my funding body.
2.1.1 This may also include clinical notes in some cases. We will obtain written consent from you before disclosing these notes, except in cases where consent is provided by the referring party either by way of contract or prior agreement with you.
3. Late Cancellations and Missed Appointments
3.1. I agree that if I am unable to attend a booked appointment, I will provide at least 24 hours' notice, wherever possible.
3.2. If I do not attend an appointment or cancel with less than 24 hours' notice, I agree to the following:
3.2.1. I may be liable to a cancellation or missed appointment fee.
3.2.2. For Funded or Referred patients (as described in point 2), the funding or referring party may be advised of the cancelled or missed session and this may affect continuation of your treatment.
3.3 For appointments booked with less than 24 hours' notice, points 3.1 and 3.2 do not apply. However, notice of a cancellation in these cases should be provided as soon as possible.
4. Charges and Payment
4.1 You will be charged for your appointment in line with the rate current at the time of booking. This can be obtained by contacting us.
4.2 Payments can be made using any of the following methods:
4.2.1 Online Electronic Payment
4.2.2 Credit Card
4.2.3 Debit Card
4.3 Payments can be made in advance, or will become due immediately following the appointment.
4.4 Delayed payments will be subject to our debt recovery process, including any interest accrued.
4.5 Patients are requested to check the conditions of their Health Insurance Policy and/or referring party, as we cannot be held responsible for any non-payment of claims. If insurers are unwilling to make payment, the patient will be liable for all costs.
5. Valuables
5.1. I understand that Bluebell Physio is not responsible for the loss or damage of any personal items (including valuables and jewellery).
5.2. We advise that any valuable items, such as jewellery, not be taken to or worn at our clinics.
6. Clinical Notes
6.1 I understand that I have a right to request a copy of my clinical treatment notes, which can made by contacting us.
6.2 I understand that there may be an administration charge for this service.
7. Complaints and Feedback
7.1 I understand there is a complaints procedure which is available on request.
7.2 If I am unhappy about any aspect of my experience with Bluebell Physio, I will contact Bluebell Physio so that the matter can be resolved where possible, prevented from happening again and thus further improving our Service.
